The dossier as prescribed in Article 47, Article 48 of the Law on Social Insurance 2024; Circular 25/2025/TT-BYT dated June 30, 2025 of the Ministry of Health includes: List 01B-HSB prepared by the employer with the dossier in the following cases:
Case 1: The employee or employee's child receiving inpatient/outpatient treatment must have the original or a copy of one of the following documents: Hospital discharge papers; Medical record summary; Death certificate; Confirmation of inpatient treatment; Certificate of leave to receive social insurance. In case both parents take leave to take care of a sick child under 7 years old, the certificate of leave from one of the two;

Case 2: In case the employee or the employee's child receives medical examination or treatment abroad:
Medical examination and treatment documents showing the name of the disease and the duration of treatment are issued by the medical examination and treatment facility abroad and must meet the following requirements:
+ Have a Vietnamese translation notarized or certified in accordance with the provisions of law on notarization and certification;
+ Be consularly legalized, except in cases where international treaties to which the Socialist Republic of Vietnam is a member have other provisions

Case 3: In case of force majeure due to natural disasters, catastrophes, epidemics, documents replacing the documents specified in points 1.1, 1.2 of this clause include:
+ Decision to apply isolation measures
+ Certificate of treatment
